GILBERT ARCHERY WAIVER AND RELEASE OF LIABILITY
READ BEFORE SIGNING
In consideration of being allowed to participate in any way in Gilbert Archery’s events and activities, the undersigned acknowledges, appreciates, and agrees that:
The risk of injury from archery and other known and unknown events and activities, and/or the use of the related structures, equipment, roads, land and all other real and personal property, whether owned by the archery club or others, is significant. It includes the potential for full or partial disability, permanent paralysis, and death. While particular rules, equipment, and personal discipline, may reduce this risk, the risk of serious injury does exist; and,
I acknowledge and agree that the use of archery equipment, by myself or by others on club premises, is inherently dangerous and a high risk activity; and
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Town of Gilbert, Gilbert Archery Inc., their officers, directors, officials, agents, employees, volunteers, members, guests, other participants, sponsoring agencies, sponsors, advertisers, owners and lessors of real property and personal property used to conduct the events and activities (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, TO THE FULLEST EXTENT PERMITTED BY LAW AND, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS, EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE OR COST THAT MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these events and activities and/or the use of related real and personal property as provided above, EVEN IF ARISING FROM RELEASEES’ NEGLIGENCE.

PHOTO RELEASE
Photographs and videos are routinely taken during activities and events. I release, and agree to, the use of any photos taken of myself, or my minor child, for purposes of recording and documenting activities and events and for use in promoting archery. This includes the use of the photos and videos in marketing materials, posting in social media, for use on websites, and other uses, except not for the commercial use of selling the photograph or video. With my/our signature(s) below, I agree that images of me, or my minor child, that are taken at any activity or event, or at any time while on the archery range, may be used as stated above without any compensation or additional permission.

EMERGENCY MEDICAL RELEASE
I hereby grant consent to any and all emergency care providers designated by Gilbert Archery, Inc. to provide any necessary medical care, as a result of any injury or illness, to myself and/or my child. This consent includes first aid and transportation to/from health care providers.
I also grant consent to contact the emergency contact listed below and reveal any and all information to them concerning the emergency care and other information.

ACKNOWLEDGEMENT OF RULES AND CONSEQUENCES
I have read and understand, or had read to me and explained to me, the rules that must be followed on the range. (The rules can be found below and at GilbertArchery.com/Rules) I agree that if I do not follow these rules, I may be asked to modify my behavior and/or asked to leave the property. If I am asked to modify my behavior and/or am asked to leave the property, I agree to do so immediately and any funds paid will not be reimbursed.
I understand that if I don’t immediately leave the premises, if asked, the police will be called and charges may be filed. I may also be responsible for reimbursing any class members for class time lost due to my behavior.

BASIC ARCHERY RANGE RULES
No one is to be on range unless the range is open and a range master is present. UNAUTHORIZED USE OF THE RANGE IS PROHIBITED, AND VIOLATORS WILL BE PROSECUTED.

Anyone on the premises, including the parking lot and surrounding area, must respect and listen to the range master. Anyone not listening to the range master will be asked to leave the premises immediately and may lose their right to return.
Everyone inside the fenced area is considered a participant, even if they are not actively shooting, and must fully fill out and sign all waivers and releases, and turn them in to the range master, as soon as they arrive.
Range is for commercially made recurve and compound bow use only, and only using field points. Broadhead points and crossbows are strictly prohibited. Other bow use is at the discretion of the range master.
Maximum shooting distance is 70 meters, or 75 yards (20 meters for Open Shooting). There are markers designating these distances and no target is to cross that line at any time nor for any reason.
There will be no alcohol or drug use on the premises, including the parking lot and surrounding area. Anyone who is observed to be using, or acting under the influence of, drugs or alcohol, will be asked to leave, even if they are not shooting.
There will be no smoking on the premises, including the parking lot and surrounding area.
No animals are allowed inside the fenced area nor tied to the fence.
Music is allowed only by permission of range master.
Everyone must pick up after themselves and respect the range property and equipment.
Equipment on the range is not for public use except for designated “public targets”. Do not move equipment without permission from range master.
Shooting is only to be done from the shooting line, aiming down range.
Bow hand must not be above your head at any time the bow is being drawn (sky drawing). This is an unsafe practice and is not permitted on this range.
Anyone not following the rules may be asked by the range master to leave the premises immediately and may lose their right to return. If they refuse to leave when asked to, the police will be called to escort them off the property.
